Plantar Plate Tears

Default Plantar Plate Tears

Good Afternoon all,

Would like some advice on plantar plate tears, have never treated this condition before.


57 year old female client
6 month history of left forefoot pain
Nil history of trauma
Pain while walking and standing on feet for lengths of time.

Pain through 2nd/3rd/4th/5th metatarsals

irritation and swelling of dorsal surface of PIPJ
continued pain through metatarsal plantar surface


Ultra sound diagnosis of plantar plate tears at the level of head of 2nd/3rd/4th metatarsals.

I have no idea how to strap for plantar plate tears, wasn't too sure how effective it would be with multiple tears?

Was thinking of an orthotic - EVA post to sulcus to provide a dorsiflexory force to metatarsals but also allowing plantarflexion to the digits?
I have read some literature where this is the conservative treatment, not sure whether this would hasten the progression of the plantar tears?

Please can I have some guidance?
