    The total time stretching in this study was not enough in my opinion. I have my patients do some type of calf stretch (active end-range DF, towel stretch, wall, step, etc) every hour for at least 1-2 minutes while awake AND in addition do a brief stretch prior to the "first step". In compliant patients, this works wonderfully.

    With any stretching exercises we must ask what is actually being stretched? If the plantar fascia is being supposedly stretched by extension of the hallux at the 1MTPJ with the foot in full pronation then it is acually more likely that the stretch will be directed to the flexor hallucis muscle and tendon as these are much more elastic structures than the fascia, any benefit therefore could be due to increased range into extensio at joint, not a longer fascia?

    stress shielding may be the issue and one of the reasons it appears to work

    "In addition to exercises, silicone heel pad and nonsteroidal anti-inflammatory drugs (NSAID) were added."
    So.. how do they make that conclusion? Was it the stretching.. or the heel pads.. or the NSAIDS.. or time?
