Manual therapy vs conventional therapy for plantar fasciitis

    Here is my evaluation:
    - no sample size calculation (and 15 in each group is probably not going to be adequate)
    - 30 subjects "equally divided into two groups by random sampling" - how do you do that?
    - follow up was only 3 weeks
    - no data on characteristics of the two groups (duration, weight, age; etc)
    - the conventional treatment group got ultrasound, stretching, strengthening and ice ---- hardly what I would call conventional
    - there was a massive improvement in both groups -- ???? Hawthorne effect; they needed a non-treatment placebo group; no stat differences in two parameters measured at baseline, but stat differences at follow-up --- HOWEVER, given the big improvement in both groups, is the statistical difference of any clinical significance?

    Given your academic and research background you can likely find the flaws in a research paper that are small and may have little bearing and those flaws that are significant and have significant bearing. For the lies of myself it would probably take several readings!!

    The question though is if on a single reading you are able to find the above concerns how was the project passed for publication?

    Its a crap new journal and the article probably had no or very inadequate peer review. Just look at the typos on the journals home page: http://ijmhr.org/ijpr.html
    Some of the issues surrounding this journal are similar to those that are occuring around the Journal of Clinical Research on Foot & Ankle (discussed here: www.podiatry-arena.com/podiatry-forum/showthread.php?t=85159 )

    All decent journals will not publish a RCT that does not adhere to the CONSORT statement for the analysis and reporting of RCT's - that study does not come close, so no decent journal would have published it.

    So I started reading this paper and at the very outset I found "Plantar fasciitis is a non-inflammatory
    degenerative syndrome of the plantar fascia resulting from repeated trauma at its origin on the calcaneus." This may or may not be correct but I rather thought that the suffix "itis" inferred an inflammatory state. I decided to look out the paper that is referenced for this claim but it does not seem to exist.
    "Kwong PK, Kay D, Voner PT, White MV. Plantar
    fasciitis: mechanics and pathomechanics of
    treatment. Clin Sports Med. 1988; 7(1): 119-26."

    I don't suppose anybody happens to have a copy?

    Also worth looking up Harvey Lemont's paper from JAPMA in 2003 if this is a topic that interests you.

    Thanks Griff, I got as far as the abstract but when you try to get the full paper from the journals own website it says it doesn't exist.

    Reading the paper (Well skimming it) there is one area of concern that immediately stands out for me:

    The are aspects of the treatment that appear in both protocols
    there is positive change in reported pain relief in both
    So how do we know that it wasn't the common interventions that accounted for the greatest change?
    At the end of the day it comes to the fact that stretching with manual therapy is a little better than just stretching ; it doesn't really say much about the effectiveness of manual therapy.
