17yo 196cm tall 75 kg athletic basketball and Aussie rules player
Mobile cavus foot type with Hx of knee and impact related symptoms using orthoses 3+ yrs and is assymptomatic apart from loud frequent clicking which appears to be from 1st MPJ. 1st heads are larger than average with some dorsal enlargement but does have full ROM in gait and unweighted. Articular surfaces feel smooth, no pain on traction or comression through full range.
Am getting some plain films but as yet have not got them.
It's not about pain or lack of function it's about the noise....what to do?

I don't treat gait sounds, unless it is symptomatic since the treatment is often worse than the disease.

I realize that you are trying to help your patient. However, other than a possible orthosis modification such as a Morton's extension to limit first mpj dorsiflexion or 1st MPJ strapping to immobilize the joint that may, in turn, cause other problems, how invasive of a treatment are you willing to propose for the patient to silence his feet? In other words, as I said before, is the treatment going to be worse than the disease?
