CPD Questionnaires

    I am just wondering if anyone has any idea as to where I can find questionnaires to do to get points for this new CPD system, under sub heading (Further Education).

    And does anyone know if a certificate is provided at the end of the online questionnaires to be printed myself?

    I would like to tag onto Clements question on questionnaires and do you have samples; verbal, printed and internet:

    How are you are evidencing patient advice and information:

    1. Prior to visit
    2. The Visit
    3. The treatment Programme
    4. Follow-up visit
    5. Aftercare/Education

    Seeking continual feedback from patients:

    1. Undertaking regular surveys on satisfaction and improvements
    2. Evidence how feedback is used

    Any help would be appreciated.
