Default orthotic paperwork

Podiatry Arena members do not see these ads
Hello I am a new graduate, this is my 1st post...
I wish to try and make my own Orthoses from scratch to casts, eva polyprop etc however am having difficulty finding out what I need to record in the patients notes for these bespoke type devices. Some people I have spoken to just write "eva device to cast" surely you need more than this.
The casts will be taken in clinic setting but the orthoses imade in my lab (shed)
I understand that there would be Health and safety as well to sort re glue storage and use as well ??, but it is how to write the manufacture up that is my concern.
Also do I need to register premises etc.
My clinic is in the UK.
Any direction advice would be much appreciated.
