Default Clicking Tendo-achilles

Podiatry Arena members do not see these ads
36 YO patient has a clicking sensation at the TA above the ankle joint not painfull does not happen on dorsiflexion or plantarflexion of the foot only when wt bearing and leaning forward what is this ????

I am assuming this is tendonitis as the pt is a runner if this is the case what can he do ??

I have had a patient with clicking in the achilles tendon but has only been painful with exercise...pain would usually suggest there to be some pathology, you could argue if it's not sore, in this situation, do you need to treat it!?

In my patients situation there was some mild thickening of the tendon. I would usually suggest for any achilles tendinopathy the Alfredson regieme (sp.?), where by they should use eccentric drop-downs off a step and adequate stretching of the gastroc and soleus muscles. The article suggests 3x15 reps twice a day for 12 weeks. Biomechanical correction should be used via orthoses and appropriate footwear.

If you would a mor eindepth explaination of the Alfredson article or the reference reply and I can explain.

It sounds possible that there is an exostosis or a loosening of the tendon sheath.I would send the pt for an MRI to see what is up,then I would go from there.Maybe a high top sneaker would help.I would consider a period of RELATIVE rest.
